Prepare all required documents:

    • stamp duty of euro 16;

  • PERSONAL DATA:

    • Copy of your passport (only the pages showing personal data, expiry and entry stamp);
    • Copy of your residence permit or declaration of presence (to be done at the Police headquarters within 8 days from entry into Italy if there is not stamp of the Italian border authorities on your passport);
    • Copy of the residence permit of the FAMILY MEMBER on whom you will be dependent;
    • In case of children aged under 14 a copy of their passport is required, and also of their residence permit.

  • IN YOUR COUNTRY OF ORIGIN prepare THE TRANSLATED AND LEGALISED version of:

    • Personal documentation proving the family relationship (birth or marriage certificate) to be translated and legalised or apostilled at the Italian Embassy/Consulate of your country of origin.
      • in case of underage children, the authorization of the other parent is required;
      • Parents must submit documents proving that they don’t have other children in the country of origin or documents proving that adult children have severe disabilities;
      • parents aged over 65 are obliged to register in the National Health Service or to take out a private health insurance.

  • ACCOMODATION:

    • Self-certification of family member’s residence and family status;;
    • Declaration of hospitality including copy of the HOST’s proof of identity;
    • HOUSE-OWNER’s authorization including a copy of his/her proof of identity (Identy Card ID);
    • Duly registered rent agreement or property agreement;
    • Proof of SUITABILITY OF ACCOMODATION certified;

  • INCOME:

    • FAMILY MEMBER's last income declaration (CUD or 730 or UNICO form)
      • For employees:
      • Family member's last three payslip or INPS payment slips;
      • Employment contract
      • For self-employed people:
      • Company’s updated financial statements;
      • Visura camerale.

IF YOU DON’T HAVE ALL THESE DOCUMENTS, YOU ARE NOT ALLOWED TO PROCEED!!!

Owner’s authorization including a copy of his/her proof of identity

If your family member taking charge of you as dependent family member is:

  • Self-employed, he/she should turn to their accountant, and ask for updated financial statements.
  • Employee, he/she must turn to their employer or to any workers’ association/trade union.
